I have read the provided information about the Ages & Stages questionnaires, and I wish to have my child participate in the online screening program. I understand that Oswego County Help Me Grow shares summary, de-identified developmental screening data with partners in our community and with the New York State Ages & Stages Hub to assist in improving the early childhood system for children and families. I will fill out the questionnaire about my child's development and promptly submit the completed questionnaire through this Family Access online questionnaire completion system.
