Welcome to our ASQ Online screening program! 

The first five years of life are critical for your child’s brain development and sets the stage for success in school and later in life. It is important to keep track of your child’s development to help give them the best start during these early years. Ages & Stages Questionnaires®, Third Edition (ASQ®-3) and Ages and Stages Social Emotional Questionnaires (ASQ:SE-2) are offered in Delaware and rely on YOU, the parent, as experts. These 15-minute questionnaires are easy to use and create the snapshot needed to catch delays and celebrate milestones.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires, Third Edition (ASQ-3), to help you keep track of your child's development. The questionnaire may be provided every 2-, 4- or 6-month period. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

By giving my consent, I understand that ASQ screening results will be shared with:

• The Delaware Department of Education (DDOE) and the Division of Public Health

   (DPH) within Delaware Health and Social Services (DHSS) to better support my child.

• My child care program who is required by law to track completed developmental

   screenings to help better support my child.

• The Birth to Three Early Intervention Program (B23) or my school district of residence

   and/or school district where my child care is located because they review the

  screenings and help with next steps.
