Welcome to our ASQ Online screening program!

The Rock Island County Regional Office of Education provides this online screening service to help provide the best possible start for all children in our community. As part of this service, we provide the Ages and Stages Questionnaire (ASQ-3), which tracks your child’s cognitive development, as well as the Ages and Stages Questionnaire (ASQ-SE) to track your child’s social emotional development.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires: Social-Emotional, Second Edition (ASQ:SE-2) to help you keep track of your child's social and emotional development. The questionnaire may be provided for use at 2, 6, 12, 18, 24, 30, 36, 48, or 60 months of age. You will be asked to answer questions about some of your child's behaviors. The questionnaire includes questions about your child's ability to calm down, take direction and follow rules, communicate, perform daily activities (e.g., eating, sleeping), act independently, demonstrate feelings, and interact with others.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

The Rock Island County Regional Office of Education collaborates with many early childhood programs and services in the community to help them provide high quality programs for children and families, and support your child's development and learning. RIROE and BHASED will review your child's information in an effort to support his or her development, including ways to meet any developmental needs. The information you enter into this website is confidential, secure and will not be seen or accessed by anyone other than the Rock Island Regional Office of Education and BHASED without your further permission. These agencies will not share any information that identifies you, or your child, with any other person, organization, agency or school without your further permission.

As part of this effort, information may also analyze the aggregated data of the children participating in this program to help us understand how many children are being screened, the overall developmental needs of children in our community, and what additional supports and services they are eligible to receive and what services are still needed. Your child's identifying information is removed from the data immediately upon transfer. The RIROE maintains the strictest confidence with all of your child's information, and shall follow all laws and regulations relating to your and your child's privacy rights.

 

I have read the provided information about the Ages & Stages Questionnaires, and I wish to have my child participate in the online screening program. I will fill out the questionnaire about my child's development and promptly submit the completed questionnaire through this Family Access online questionnaire completion system. By completing the Questionnaires, I agree to:

1. Share responses about my child with the Rock Island County Regional Office of Education and BHASED.

2. Give permission to aggregate my responses about my child, and any follow-up activities and share this information with community stakeholders.

The permission you have granted by submitting the completed questionnaires will end within one year from the date of the last online screening completed through this program, unless otherwise revoked in writing to the RIROE.
