Welcome to the Help Me Grow Vermont’s Social and Emotional Development Screening Program!

Because your child's first five years of life are so important, we want to help you provide the best start for your child. You've been invited to complete a brief screening tool, the Ages & Stages Questionnaires, Social-Emotional, Second Edition (ASQ:SE-2), which offers a fun and interactive way for you to learn about and promote your child’s social and emotional development. The questionnaire may be offered every 6, 12, 18, 24, 30, 36, 48, or 60 months of age and will provide a quick check of your child’s development. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's ability to calm down, take direction and follow rules, communicate, perform daily activities (e.g., eating, sleeping), act independently, demonstrate feelings, and interact with others.

Help Me Grow Vermont will communicate screening results and other basic information about your family with your child’s health care provider and other service providers. When appropriate, a copy of the scored questionnaire in full will be shared with your child’s health care or other service provider to improve support for your family.  

Additionally, HMGVT shares developmental screening information with other entities to provide an overall view of how Vermont’s children are doing and to coordinate support. By clicking “Submit” you are agreeing to share your child’s information and screening results as outlined in the Authorization for Release and Disclosure of Developmental Screening Results. This authorization is voluntary.
