With a quick 10 minute look at your child's latest developmental accomplishments, using an Ages and Stages Questionnaire (ASQ),  you will have a better idea of how a child learns and grows.   Once completed, you will receive by email:

* Summary of your child's ASQ results
* Fun activities to do with your child to support his/her continued development
* Recommendations for community resources 
* If necessary - Referral to Early On or your school district for further evaluation.  Both offer FREE evaluation and local resource that may help with your child's development.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires, Third Edition (ASQ-3), to help you keep track of your child's development. The questionnaire may be provided every 2-, 4- or 6-month period. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

I have read the provided information about the Ages & Stages questionnaires, and I wish to have my child participate in the online screening program. I will fill out the questionnaire about my child's development and promptly submit the completed questionnaire through this Family Access online questionnaire completion system.

By completing an ASQ developmental screening for your child, you are authorizing verbal and/or written communication between Great Start Livingston  and Early On and/or the support staff in your school district.  Great Start Livingston will refer your child to Early On (for children 2 months - 2 years/9 months) or to the support person in your school district (for children 2 years/9 months - 5 years) if your child's score indicates a possible area(s) of concern.    Both are free local resources/services.  You will have the opportunity to have a free evaluation for your child and free supports if he/she qualifies.  

Your child's results are confidential and will not be shared outside the Great Start Livingston screening project.

Your child's non-identifying information will be combined with others in the program and data will be studied to help improve access to services and supports for all children in Livingston County.
