Please note that all the information you enter is secure and cannot be seen or accessed by anyone other than the program staff who have invited you to participate in this screening.

 

Please enter your child's birth date below.

 

If your baby was born more than 3 weeks early, enter that number in the box that says Weeks Premature. If you know the number of weeks gestation they were but are unsure of how many weeks preterm that is, subtract that number from 40. For instance, if your baby was born at 29 weeks gestational age, subtract 40 minus 29 = 11 weeks early. Then Enter "11".

 

 

 

Once you’ve entered that information the questions appropriate for you baby’s age will appear.
