Welcome to our ASQ Online screening program!

Your child's first five years of life are the most crucial to their future health and wellness. You've been invited to participate in the Ages & Stages Questionnaires, Third Edition (ASQ-3), to help you keep track of your child's development. By participating in the questionnaire you will provide valuable information about your child's growth and development. You will be provided information about your child's strengths and areas in which your child may need more help or practice. The answers you provide to the questionnaires will be hlpful in determining whether your child needs further support or assessment. Your answers will also help the Judy Center get to know your child better and how to support them in the playgroup and Little Learners programs. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills. You will be asked to answer questions about some things your child can and cannot do.

To get started, please enter your child's birth date and the number of weeks he or she was born premature below. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the Judy Center staff members who have invited you to participate in this screening.
