The Down East Partnership for Children is providing parents with free access to this online screening tool.

Young children, birth to 5 years, are growing and changing at a dynamic rate. Sometimes parents of young children are concerned that their child may not be developing in areas such as speech, physical ability, social skills, and problem solving.  Parents or other caregivers can use the Ages and Stages Questionnaires- Social Emotional ® (ASQ: SE-2®) to track their child’s social and emotional development and find out if they are meeting their developmental milestones on time or if there is a delay. Children from 1 months through 5 years of age, living in Edgecombe or Nash Counties, are eligible to participate in the ASQ: SE-2® screenings.  The sooner you address a developmental delay or special need, the better long-term success you can expect for your child’s overall well-being. Early intervention is key. The questionnaires are quick and easy to fill out and can provide valuable information about your child’s developmental progress. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires: Social-Emotional, Second Edition (ASQ:SE-2) to help you keep track of your child's social and emotional development. The questionnaire may be provided for use at 2, 6, 12, 18, 24, 30, 36, 48, or 60 months of age. You will be asked to answer questions about some of your child's behaviors. The questionnaire includes questions about your child's ability to calm down, take direction and follow rules, communicate, perform daily activities (e.g., eating, sleeping), act independently, demonstrate feelings, and interact with others.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who will be reviewing this screening.

The Ages & Stages Questionnaires (ASQ: SE-2®) are designed for children birth through age five. If your child is age six or more, please discuss his or her development with your child's health care provider, your local school district, or your child's teacher.
