Screening young children is an effective, efficient way for families to catch delays and start treatment when it does the most good—during the crucial early years when the child’s brain and body are developing so rapidly.

Not every disability is visible to the eye. Developmental delays and disabilities such as autism, emotional disturbances, and speech and language disorders often go undetected until a child enters elementary school.

If you have concerns regarding your child's development, please take the time to complete the assessment. If your child is receiving services for a special need, they have already been screened and an additional screen is not necessary.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires, Third Edition (ASQ-3), to help you keep track of your child's development. The questionnaire may be provided every 2-, 4- or 6-month period. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

By completing the (ASQ-3) you are giving Lenawee Great Start permission to share your child's results with other professionals so that we can provide relevant support for your child. All information regarding you and your child will be kept confidential. Once you have filled out a questionnaire, you will receive the results within a week. You will receive a letter with your results along with activities specific to your child’s development. If your child’s scores indicate concerns with development, or if you have noted concerns, we will let you know and help you get in touch with the appropriate resources.
